Transitions: Supporting Adolescents Before, During, and After Higher Level of Care Treatment

Description

Transitions to and from higher levels of care are among the highest-risk periods for adolescent patients, yet they're often the least standardized part of treatment. This training walks mental health providers through the full arc of that transition, including preparing an adolescent and their family for step-up to a higher level of care, staying clinically connected while the patient is in treatment, and supporting with a discharge plan that protects against relapse and readmission during the vulnerable weeks that follow. Drawing on developmental considerations specific to adolescents and practical tools providers can use immediately, this session is designed to close the gap between providing treatment and managing the transitions around it.

Learning Objectives

Participants will be able to:

  • Identify at least three red flags that signal a need for a higher level of care and initiate an effective warm handoff to the receiving treatment team.

  • Describe two concrete strategies for maintaining clinical continuity and family support while an adolescent patient is engaged in a higher level of care.

  • Identify three components implement a discharge and step-down plan that reduces relapse and readmission risk during the high-vulnerability post-discharge window.

Educational Goal

The educational goal of this workshop is to equip mental health providers with practical, actionable strategies for managing each phase of an adolescent's transition into, through, and out of a higher level of care. Participants will leave with tools — including handoff practices, recommended communication touch points, and discharge planning components — they can implement immediately in their own clinical settings.

Presenters

Elizabeth has been working with Center for Discovery since 2015 and has held previous roles of Program Therapist, Program Director, Regional Director, and Regional Clinical Director. She is currently a Clinical Training and Development Specialist. Elizabeth is licensed as a Professional Counselor and Nationally Registered Dance/Movement Therapist. Elizabeth's role now focuses on training compliance and development, clinical key performance indicators, and ensuring clinical quality and integrity in our programs. She enjoys providing supervision and consultation to clinical leaders and therapists and believes that connection and collaboration allow the space for joy to reignite our reasons for why we do the work that we do. Elizabeth believes in the importance of providing a safe place for patients to foster and strengthen healthy relationships to support their journey towards real life recovery, and that we are best able to do this as providers when we are feeling connected to our "why".
Erica Waldron (She/Her) is a Licensed Mental Health Counselor with over a decade of clinical expertise in the eating disorder and mental health field. With a Bachelor’s degree in Psychology from Sacred Heart University, as well as, a Master’s degree in Counseling Psychology from Palm Beach Atlantic University, Erica's journey extends beyond academia. As a former four-year Division I Field Hockey Athlete, she earned accolades for her athletic prowess and embodies a spirit of compassion, tenacity, and teamwork that remains unwavering.

Throughout her professional journey, Erica has assumed diverse roles as Program, Clinical Director and now Clinical Supervisor with Center for Discovery. Erica has undergone EMDR training, is a Qualified Supervisor, a Certified Eating Disorder Specialist Consultant.
