Describe the basic elements of the 4th Ed. ASAM PPC, including the different types of Assessments, Levels of Care, Dimensions of Care, Sub-Dimension Drivers, and Algorithm for Placement.

Discuss how to use the ASAM elements to create interpretative summaries, treatment plans and reviews, continuing care plans, and transition and discharge summaries.
Educational Goal
Attendees will increase their ability to engage in effective case conceptualization, case management, and documentation when working with Substance Use Disorders (SUD) and Co-occurring Mental Health Disorders (COD).
Description
This segment introduces the American Society of Addiction Medicine’s (ASAM) 4th edition Patient Placement Criteria (PPC), including how to use this frame of reference to conceptualize a client’s needs, strengths, and areas for development and to engage in collaborative treatment planning. This segment will explore using these criteria as a guide to produce clear and concise documentation that summarizes assessment findings, supports the plan of care and its ongoing implementation, and revision. Additionally, this segment will explore the decision-making regarding and subsequent documentation of the transition to the next level of care. Throughout the training, emphasis is placed on clinically appropriate language for the medical record.
NOTE: If you have a copy of the 4th Ed. ASAM Patient Placement Criteria available, this would be a useful but not necessary tool for the training.

Presenters

Cinde Stewart Freeman is the Chief Clinical Officer at Cumberland Heights in Nashville, TN. Cinde has been in the behavioral healthcare field for over 35 years, serving in direct care, clinical management, quality management, and leadership roles. She is a bachelor’s prepared Registered Nurse and a master’s prepared Level II Licensed Alcohol and Drug Abuse Counselor. Cinde has also held the Qualified Clinical Supervisor credential for many years and enjoys stoking the passions of people who are coming into the field. She has trained at the national, state, and local level, holding a particular interest in the relationship between somatic and spiritual healing as well as how the lived wisdom of the Twelve Steps informs clinical education and experience.

Marlena has been at Cumberland Heights since 2012 and currently serves as the Director of Utilization Review Services. She holds a BS in Human Development from Vanderbilt and an MSW from Edinboro University of Pennsylvania. She holds LMSW and LADAC II licenses in the State of TN and is a QCS supervisor for those seeking LADAC licensure. She lives in Nashville with her Jack Tzu (Jack Russell/Shih Tzu mix), Maggie, and believes there is no such thing as unusual behavior if you know someone’s whole story.

Matthew has worked at Cumberland Heights for over five years and worked in a variety of clinical positions and programs since his start. He has been a fundamental member of the Nashville recovery community by hosting events and creating more resources for those suffering from the disease of addiction.
