This workshop equips pediatric providers with practical, clinic-ready strategies to strengthen integrated care delivery using the medical home model. The session emphasizes actionable approaches to improve outcomes for children and youth, particularly those with behavioral health (BH) needs, by advancing care coordination, team-based care, and family engagement.

Presenters
Tiffany Williams, MSW, RSW is currently the Care Coordination Coordinator for the State of Louisiana with the Bureau of Family Health (BFH). She joined the Bureau of Family Health in 2012 as a Care Coordinator and implemented an evidence-based care coordination model in three Louisiana pediatric academic clinics. In her role as Care Coordination Coordinator, Tiffany provided practice level care coordination services to several Pediatric Clinics across the New Orleans area, trained medical providers, clinic staff, residents and students. Tiffany also served on LSU’s Interdisciplinary Autism Team from 2015-2022. In her position as the Care Coordination Coordinator, Tiffany supports the BFH Family and Community Systems Team, Communications Team and Clinical Team on development of CYSCHN resource materials. Tiffany supports the Title V Medical Home didactics, provides FRC services for the Birth Defects Monitoring Program and is the lead Faculty for the Medical Home Case Presentation Activity. Tiffany earned both a Bachelor’s and a Master's of Social Work from Southern University at New Orleans (SUNO) with a concentration in Administration Planning Organizing/Health & Mental Health. Prior to this position, she was a Director of Admissions and Social Services for a local nursing home. For the past 26 years as a social worker, she has worked with families across several different spectrums.
