Description
Providing behavioral health and medical care to clients with complex needs often involves several teams and specialties, from those providing care to those handling administration, to those providing social needs assistance. With so many competing priorities and responsibilities, teams often cluster together without communication and input from one another. This session will review aspects of a fully integrated system that reduces friction between teams, redundancies in processes, and improves the quality of care for clients. Specifically, participants will work together in small groups to understand what has contributed to fragmented care and develop a seamless client journey that improves efficiency and addresses the totality of each client’s needs.
