Description
This session will explore the gap between behavioral health quality expectations and the realities patients and care teams face across healthcare settings. Drawing from experience in both managed care quality and frontline inpatient social work, the presentation will examine common barriers to follow-up, engagement, and continuity of care particularly within Medicaid populations. Participants will gain practical strategies to strengthen care transitions, improve interdisciplinary collaboration, and support better behavioral health outcomes in real-world practice.

Identify at least three common barriers that impact behavioral health follow-up, patient engagement, and continuity of care within Medicaid and high-risk populations.
